There are three different EPF composite claim forms they are Form 19 for Aadhar card holders, Form 19 for Non Aadhar card EPF account holders and Form 20. Form 20 is a composite claim form which is provided by the EPFO to claim the EPF funds in the death cases. Form 20 is applicable for the EPF account holder nominees to claim the EPF returns upon the death of the EPF account holder. How to Fill EPF Form 20 Composite Claim Form Online & Offline?

EPF Composite Claim Form 20 is a PF claim form which is used to claim the PF funds for Partial withdrawal and for the EPF final Claim by the nominees in the death cases of the EPF members. Form 20 Claim is for the nominees to claim the PF funds on the death of the EPF account holder.

  • EPF Composite Claim Form 20 consists of 14 fields which are mandatory to fill by the applicant at the time of applying for withdrawal of Provident Fund or Pension or Insurance.
  • Tick the correct box whether you are applying for withdrawal of Provident Fund or Pension or Insurance EDLI.
  • Enter the name of the diseased member in capital letters.
  • Enter the diseased member Father name or Spouse’s name.
  • Enter the marital status of the diseased member.
  • Enter the Aadhar number if it is allotted.
  • Enter the Universal Account Number UAN of the diseased member if available otherwise enter the PF account number.
  • Enter the date of leaving the service.
  • Period of Non contributory service of the diseased member – this should be filled by the employer.
  • Enter the date of death of the member.
  • Mention whether the member died in service or not.
  • Enter the Claimant details, Spouse name, Aadhar number, gender, date of birth, martial status, relationship with the member for claiming the Provident Fund, Pension, and Insurance (EDLI) in 11th Field.
  • Enter the children details.
  • Enter the bank account details of the claimant for the payment PF & EDLI in 12th field.
  • Enter the bank account details or attach a cancelled cheque.
  • Enter the bank name, Bank address, Branch name, and IFSC code.
  • Enter your full postal address.
  • Enter the bank account details of the claimant for Pension in 13th field.
  • Enter the bank account details or attach a cancelled cheque.
  • Enter the bank name, Bank address, Branch name, and IFSC code.
  • Enter the full postal address.
  • After entering all the details check once and sign the application form with date.
