The Government of India is providing 3 types of EPF Application forms through EPFO, they are

  • EPF Claim Forms
  • EPF Registration Forms
  • EPF Return Forms

EPF Claim Form/ EPF Withdrawal Form:

EPF Claim form can be used to both claim Final EPF, EPF Partial Withdrawal, and EPS withdrawal. There are different types of EPF Claim forms, these EPF claim forms are classified basing on the EPF account holder’s requirement.

  • Composite EPF Claim Form for Aadhar card holders or Form-19.
  • Composite EPF Claim Form for Non-Aadhar Card or Form-19.
  • Composite EPF Claim Form in Death Cases or Form-20.
  • Employees Pension Scheme Forms.
  • Employees Provident funds Scheme Forms.
  • General Forms.

Composite EPF Claim Form (Aadhar)/ FORM NO – 19:

Composite EPF claim Application form is applicable for the EPF account holders who have attached their Aadhar number to the EPF account. EPF account holder can claim EPF final withdrawal, EPF partial withdrawal and withdraw the refund of Employers share or EPS.

EPF Composite Claim Form/ FORM NO – 19 for Non-Aadhar card holders:

EPF Composite Claim form for non-Aadhar card holders is applicable for withdrawing the EPF. This EPF claim form is useful only for the EPF account holders who have not linked their Aadhar card to the EPF account. By using this Application form, EPF member can claim final withdrawal, partial withdrawal and withdraw the refund of Employers share.

Composite EPF Claim Form in Death Cases/ FORM – 20:

Composite EPF Claim form FORM-20 is applicable to claim the EPF in the death case of the EPF account holder. Form 20 EPF Claim form is used to withdraw the EPF by the Widow/Widower/Nominee/Orphan related to the expired person.

Form 10 D (Non-Aadhar) /Employees Pension Scheme:

To apply employees pension scheme Form 10 D is required. EPS registration form or Form 10D is applicable for applying the Monthly Pension under Employees Pension Scheme who is not having Aadhar card.

Form 10 D (Aadhar) /Employees Pension Scheme:

To apply employees pension scheme Form 10 D is required. EPS registration form or Form 10D is applicable for applying the Monthly Pension under Employees Pension Scheme who is having Aadhar card.

Live Certificate and Non-Re-Marriage Certificate:

Live Certificate must be submitted at their nearest EPF office in the month of November by all the Pension beneficiaries including children. Pension beneficiaries can get the Life certificate from the bank where the pension is disbursed or they can get it by visiting their nearest EPF office.

Non-Re- Marriage Certificate:

Non –re-marriage certificate must be submitted by the widowed pensioner at the time commencing of pension. This certificate should be submitted only by the widowed pensioner at the time pension commencement.

FORM 13: Employees Provident Fund Scheme:

FORM 13 is applicable to transfer the existing Provident Fund account from one company to another. For example, if you have changed your company then in such a case, by using Form 13 you can transfer your PF account from the old company to the new company.

FORM 14 Employees Provident Fund Scheme:

Form 14 is applicable for financing a Life Insurance Policy from the existing Provident Fund Account. Form 14 is applicable for the PF account holder who wants to make an LIC policy from the PF account funds.

EPF RETURN FORMS for Exempted Establishments:

 FORM 3 (PS):

FORM 3 is consolidated return given by the employees who are entitled to become the members of the pension fund within the 15 days of coverage.

FORM F1:

FORM F1 is consolidated return given by the employees who are entitled to become the members of the Insurance fund within the 15 days of coverage.

FORM F5 (PS):

Form F5 is consolidated return to be given by the employees who are leaving the services.

FORM F2 (IF):

Form F2 is consolidated return to be given by the employees who are entitled for the membership of the Insurance Fund

FORM F3 (IF):

Form F3 is consolidated return to be given by the employees who are entitled for the membership of the Insurance Fund while leaving the services

FORM F7 (PS):

Form F7 is applicable for showing the month wise recoveries in respect of one PF member for the current financial year by the employer. Form F7 must be submitted by 30th April of the current year

FORM F8 (PS):

This is a consolidated Annual contribution Statement showing the each and every members PF contribution and must be submitted by the employer by 30 April of every year

FORM F4 (IS):

This is a consolidated Annual contribution Statement showing the each and every members Insurance Fund contribution and must be submitted by the employer by 30 April of every year

FORM – F9:

Form – F9 is the declaration given by the employee during the time of his/her employment in which the employees’ pension scheme is in force.

EPF RETURN FORMS for UN-Exempted Establishments:

FORM-1(Revised):

Form-1 is applicable for the EPF account holders for giving the details of the family members & it is used to for releasing PF dues to the Nominees

FORM-9(Revised):

Form-9 is a consolidated return of employees who are entitled to the Provident Fund & Pension Fund

FORM 5A:

Form 5A is applicable for the employers who intend to return their ownership and it should be submitted to Regional Commissioner

FORM 12 A:

Form 12 A is a consolidated statement of dues and remittance.

FORM 5:

Form 5 is applicable for the employees who have qualified for the membership in Employees Provident Fund for the first time.

FORM 10:

Form 10 is applicable to the EPF account holders to make the returns of the service while leaving.

FORM 3A:

Form 3A is applicable for the employer and it is like a Member’s annual contribution card for submitting the month wise recoveries towards Employees Provident Fund and Employees Pension Scheme for the current financial year. Form 3A must be submitted by the employer before 30 April of the current financial year.

FORM 6A:

Form 6A is a consolidated annual contribution of each member of the establishment and it must be submitted by the employer by the 30 April of the current financial year.

FORM 11:

FORM 11 is applicable to the give the declaration by the employee during the time of employment in any establishment on which EPF scheme is applicable.

EPF Registration Form/ FORM 11(Revised):
